Player Accident Insurance (PAI)

Futsal 4 Life (F4L) offers a level medical reimbursement (via PAI) for non-Medicare expenses incurred from injuries sustained while participating in F4L leagues.

The amount of PAI cover is not ‘comprehensive.’ As a result, we strongly recommend players take out their own private health cover, that is tailored to their own personal situation.

Insurance claim steps:

  1. Players who are eligible for an insurance claim, must have incurred the injury whilst playing in a F4L league.

  2. To initiate process: Injured player to contact insurance broker (details at bottom)

  3. Broker will deal with the application (not F4L) by discussing your claim.

  4. Should broker agree your application could fit within the insurer's policy criteria, he will provide forms to apply for medical reimbursement.

  5. Injured player to send insurance application docs and relevant medical info to broker.

  6. The broker then requires F4L confirmation that the player was a member and injured during F4L leagues.

  7. Broker will send all documentation to insurer and after a period of time the broker will inform injured player of the outcome.
